CURRENT POLICY PERIOD
Individual Licensee Coverage
Individual Enrollment Form (Prorated): Purchase insurance through the group program for individual (rather than firm) licensees with an individual effective date after January.
Backdate Request Form: Request to backdate the inception of your individual policy period up to 90 days from the date we receive the completed request form and applicable premium.
Endorsement Request Form for Current Insureds: Request to add an endorsement for an individual (rather than firm) licensee who is already enrolled in the current group program.
Optional Firm Licensee Coverage
Firm Enrollment Form (Prorated): Purchase insurance through the group program for firm (rather than individual) licensees with an individual effective date after January.
If your previous coverage expired January 1, and you have any break before your new inception date, you will have a break in coverage and lose any previously established retroactive date, which results in loss of prior acts coverage. If you do not have continuous coverage, your new retroactive date will be the effective date of your new coverage. In addition, if you have a break in coverage and are not covered during a period that your license is active, you will not comply with the licensing requirements for your state and your license may be placed inactive and/or other action may be taken against your license.
If you did not renew timely, you may qualify for reinstatement of your coverage for 90 days after your coverage has expired. Until March 31, you may request a January 1 effective date by mailing a check or money order for your premium payment with a completed Backdate Request Form. The carrier reserves the right to deny requests to backdate. Additionally, backdating does not cure failure to comply with state law, so you may still be subject to state regulatory penalties and fines.
After March 31, if you would like to request to backdate coverage a maximum of 90 days, please refer to the Prorated Premium Chart to determine the correct amount to send with your Backdate Request Form. Always pay your premium on time to avoid a break in coverage, protect yourself from uncovered claims and avoid penalties and fines.
