To obtain
coverage, print and complete an Enrollment Form and mail along with premium
amount made payable to RISC. If you are a new licensee enrolling after the
original renewal period, please refer to the Prorated Premium Chart to
determine the correct premium amount.
2012 Insurance for Individual Licensees
Enrollment Form (Prorated):
Please use
this form if you are an individual (rather than a firm) licensee and want to
purchase insurance through the group program.
Reinstatement Form:
Individual
(rather than firm) licensees who want to request an effective date in a
month prior to the date of purchasing insurance through the group program
must use this form.
If approved, the effective date may be backdated up to 90 days. Please note
that backdating the effective date of your insurance does not cure any
failure to comply with the state’s mandatory insurance requirements.
Endorsement
Request Form for Current Insureds:
Individual
(rather than firm) licensees who have purchased insurance through the
current group program and wish to add an endorsement(s) to their existing
policy
must use
this form.
IMPORTANT
NOTICE:
If your
previous coverage expired January 1, you should request an effective date of
January 1. This will ensure that you maintain continuous
coverage. Otherwise, you will have a break in coverage and lose any
previously established Retroactive Date (for prior acts coverage). If you do
not have continuous coverage, your new Retroactive Date will be the
effective date of your new coverage. In addition, if you have a break in
coverage and are not covered during a period that your license is active,
you will not comply with the licensing requirements for your state and your
license may be placed inactive and/or other action may be taken against your
license.
If you
discover that you have missed timely renewal, you may qualify for
reinstatement of your coverage for 90 days after your coverage has expired.
If you would like to reinstate coverage to the inception date of the current
group policy within the 90-day period (1/1 - 3/31), you may send your
premium payment via check or money order in the mail with a Reinstatement
Form. Reinstatement will eliminate a break in coverage and loss of your
Retroactive Date.
If you
would like to backdate coverage a maximum of 90 days after the 90-day
reinstatement period has passed, please refer to the Prorated Premium Chart
to determine the correct amount to send with your Reinstatement Form.
This will avoid a break in
coverage and loss of your retroactive date. However, this procedure
does not impact the licensee's failure to comply with the mandatory
insurance guidelines established by the Commission and you may still be
subject to penalties and fines. Always pay your premium on time to avoid a break in coverage and to protect
yourself from uncovered claims and noncompliance with licensing
requirements.
