IMPORTANT NOTICE: 

The IREC may inactivate your license and assess penalties and fines up to $5,000 if you do not obtain or renew coverage for the October 1, 2012 to October 1, 2013 period by 11:59:59 p.m. MDT on September 30, 2012. 

In addition to violating state law and incurring fines and penalties, failure to enroll by September 30, 2012 at 11:59:59 p.m. may cause you to lose coverage for prior acts, a valuable feature of the policy.  Your “retroactive date” determines whether there is coverage for acts that occurred prior to the policy’s effective date.  The retroactive date is the date you first obtained, and since which have continuously maintained, E&O insurance, with no gaps.  Even a one-day gap will result in loss of coverage for any services provided before and through the last day of the gap, even if insurance was in place at the time of the services.  If you or your firm failed to enroll timely, see the information next to the Form to Request to Backdate Your Individual Effective Date or the Form to Request to Backdate Your Firm’s Individual Effect Date above.  Always pay your premium on time to protect against uncovered claims, fines, and penalties.

Enroll by Mail:  Select the form below that applies to your or your firm’s situation (note, the first three forms are for individual licensees and the last three forms are for firm licensees), complete it, and mail the completed form to either of the addresses below with the applicable premium by check or money order made payable to RISC. 

Individual Licensee Coverage
Group Policy Period: Oct 1, 2012 - Oct 1, 2013

Individual Enrollment Form (Prorated)  For individual (rather than firm) licensees wanting to purchase insurance through the group program with an individual effective date that is after or no more than 30 days before the date of submission. 

Endorsement Request Form for Current Insureds: Individual licensees who are enrolled in the current group program and wish to add an endorsement(s) to their policy must use this form.

Firm Licensee Coverage
Group Policy Period: Oct 1, 2012 - Oct 1, 2013
